Value-based care team model goal is to minimize hospital admits/emergency department utilization for our complex patients. - Type of Facility: Outpatient clinic (primary care) - Scrub Color: Pending - Charting: EPIC (experience preferred but not required) ----- JOB TITLE TAGS DEFINITIONS: Temp to Perm (LOCALS ONLY) The job is strictly for local candidates within a 50-mile radius.
