The Community Subacute and Transitional Care Center is a 106-bed skilled nursing facility (SNF) comprised of two unique and distinct units: Subacute and Transitional levels of care. This facility provides inpatient care for chronic subacute conditions and other complex illnesses in addition to the extended skilled nursing level of care. As a distinct part SNF of our Level I Trauma Center Community Regional you will be integral to the continuity of care across our health system.
