Riverside University Health System - Public Health is currently recruiting for a Bilingual level 1 Community Health Worker/Promotore II in Moreno Valley CA for the Enhanced Care Management (ECM) team. The main location for this position will be in Moreno Valley but employees may be asked to conduct fieldwork and/or home/clinic/hospital visitations throughout the County of Riverside. Depending on the assignment evenings and weekends will be required. This is a 9/80 schedule and will have every other Friday off.
A Community Health Worker (CHW) in the Enhanced Care Management (ECM) program has core essential functions that involve outreach navigation coordination and advocacy.
The goal is to provide culturally competent person-centered support for Medi-Cal members with complex medical and social needs with engagement in the community.

Principle duties:
Experienced working with people and have good team dynamics and communication working closely with the members larger ECM team including nurse care managers and behavioral health care managers to ensure seamless coordinated care.
Help individuals to navigate the healthcare system and become self sufficient and graduate program within a year
Engage in the community and with organizations to promote health equity program goals and objectives.
Conduct community outreach activities and help develop program information materials for distribution to organizations community groups schools and the public.
Represent the department at meetings with local state and federal program representatives.
Prepare a variety of reports records and correspondence with stakeholders.
Coordination with various public health programs and local state and federal government partners.
Track and report on events and meetings attended and outcomes.
Participate in all team collaborations and continuous quality improvement initiatives and projects.
Prepare quarterly reports and support work plan development.
Speak read and write in Spanish
Education: Graduation from high school possession of a Certificate of Proficiency issued by the California State Board of Education or attainment of a satisfactory score on a G.E.D. examination.
Experience: One year of experience as a Community Health Worker/Promotore I or equivalent non-County classification.
OR
Experience: Two years of lived experience that is equivalent to that of a Community Health Worker/Promotore I working with diverse populations in the community (e.g. women with infants and young children homelessness substance abuse persons involved in the justice system AIDS/HIV patients older adults and underserved populations) which would provide an opportunity to acquire the knowledge and abilities herein.
Community Health Worker Certification Required for both options.
CHW Cert. must be obtained within 6 months if selected and not certified.
Knowledge of: Client populations community resources techniques for group and individual interviewing and counseling; basic recordkeeping principles and practices; and computer equipment and applications related to the work.
Ability to: Address the client populations unique needs in a culturally sensitive manner; relate and communicate effectively with the community served; lean and understand the organization and operation of the assigned department and of outside departments and agencies; maintain strong working relationships with a wide range of community agencies and organizations; obtain and record accurate information for case documentation and other reports.

Pre-Employment
