Job Summary

Who are you

You are a caring professional who is dedicated to working with a team of case managers as they guide clients from all walks of life that includes but is not limited to homelessness substance use primary mental health diagnosis and/or co-occurring disorders incarcerated population. You have a passion for making a difference in your local community by providing oversight of case management services.

You will lead a team of client advocates as they attend treatment plan development and review meetings coordinate and monitor appropriateness of all services examine and recommend new programs and provide assistance in crisis situations to stabilize their clients.

You will ensure the team of case managers is providing exceptional client care consultative services and/or promote the coordination and development of mental health and/or substance abuse services within the community.

Working in a team-based environment is important to you. While working with the treatment team you will maintain a strengths-based approach as you work with your clients and families to assist with the retention of dignity and the development of positive qualities in the client.

You have:

  • GED/HSD and a combination of a minimum of 5 years of experience in the field life experience or relevant education OR
  • Masters Degree with LISW or LPCC licensure
  • A valid drivers license and the ability to maintain auto insurance as the position requires the capability to transport clients in your own personal vehicle within the community
  • The willingness to travel to FRC locations in Lima and Findlay.
  • CPR/First Aid Certification or willingness to obtain one and willingness to submit background check and fingerprinting
  • Ability to regularly use various computer apps including Microsoft Office our HRIS EHR and other online systems with multiple logins often simultaneously.
