Your role and responsibilities

In this role youll work directly with people receiving services to review their care and support plans ensuring they meet individual needs and promote independence and wellbeing. Youll be a key part of our mission to deliver high-quality personalised outcomes that are economically sustainable.

On a typical day you might:

  • assess eligibility under the Care Act 2014
  • arrange care packages tailored to individual needs
  • evaluate decision-making capacity under the Mental Capacity Act 2005
  • apply safeguarding policies and procedures
  • review existing services to ensure quality and compliance
  • complete checklists under the Continuing Health Care Framework
  • conduct phone assessments home visits and attend professional meetings.

This role involves lone working in the community and youll be supported through a comprehensive induction shadowing opportunities and ongoing training.

This is an exciting time to join our Mental Health Service within Adult Social Care. Were investing in additional capacity and introducing new roles to support transformation and improve outcomes.
