JOB SUMMARY:

We are seeking a passionate and skilled Medical Provider to join our growing team. This role is ideal for a provider who thrives in a supportive forward-thinking environment and is committed to whole-person care. You will provide comprehensive medical services foster lasting relationships with your patients and collaborate with a high-functioning care team to drive optimal health outcomes.

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits telehealth and occasional same-day appointments
• Perform comprehensive assessments order and interpret labs/imaging and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions supplements hormone therapies and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches nutritionists mental health providers and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings quality improvement initiatives and training sessions
• Support clinic operations by contributing to a positive team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner* PA-C MD or DO (or eligibility to obtain)
• Board certification in Family Medicine Internal Medicine or applicable specialty
• Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)
• Strong interest in preventive functional and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)
