As Supervisor ElaraConnect Engage Program Non-Clinical youll contribute to our success in the following ways:

Key Accountabilities:

  • Maintains patient and staff privacy and confidentiality in compliance with HIPAA requirements.
  • Supports effective and efficient systems for patient referrals ensuring proper intake eligibility verification and communication of referral information.
  • Oversees daily weekly and monthly reporting processes to ensure timely submission to leadership.
  • Provides direction and oversight to non-clinical personnel to ensure delivery of quality services.
  • Supervises staff to support coordination of patient care activities.
  • Maintains regular communication with department leaders and direct reports to support workflow and collaboration.
  • Builds relationships within the company by maintaining visibility and accountability.
  • Participates in assigned special projects.
  • Connects customers with appropriate internal partners based on identified needs (e.g. skilled care personal care alert follow-up healthcare coordination).
  • Maintains organized systems and effective communication processes to support continuity of quality patient care.
  • Assists in evaluating operations and identifying opportunities to improve efficiency and productivity.
  • Performs other duties/projects as assigned.

What is Required

  • High School Diploma/GED is required.
  • 2 years experience within Home Health is required.
  • Experience in Homecare Homebase EMR is preferred.
  • Demonstrates strong verbal and written communication skills.
  • Proficiency in MS Office Suite and data entry skills
  • Must have current drivers license reliable transportation and insurance in accordance with the laws of the state.
  • Must be able to sit stand bend lift and move intermittently and be able to lift at least 25lbs.
