As Supervisor ElaraConnect Engage Program Non-Clinical youll contribute to our success in the following ways:
Key Accountabilities:
- Maintains patient and staff privacy and confidentiality in compliance with HIPAA requirements.
- Supports effective and efficient systems for patient referrals ensuring proper intake eligibility verification and communication of referral information.
- Oversees daily weekly and monthly reporting processes to ensure timely submission to leadership.
- Provides direction and oversight to non-clinical personnel to ensure delivery of quality services.
- Supervises staff to support coordination of patient care activities.
- Maintains regular communication with department leaders and direct reports to support workflow and collaboration.
- Builds relationships within the company by maintaining visibility and accountability.
- Participates in assigned special projects.
- Connects customers with appropriate internal partners based on identified needs (e.g. skilled care personal care alert follow-up healthcare coordination).
- Maintains organized systems and effective communication processes to support continuity of quality patient care.
- Assists in evaluating operations and identifying opportunities to improve efficiency and productivity.
- Performs other duties/projects as assigned.
What is Required
- High School Diploma/GED is required.
- 2 years experience within Home Health is required.
- Experience in Homecare Homebase EMR is preferred.
- Demonstrates strong verbal and written communication skills.
- Proficiency in MS Office Suite and data entry skills
- Must have current drivers license reliable transportation and insurance in accordance with the laws of the state.
- Must be able to sit stand bend lift and move intermittently and be able to lift at least 25lbs.
