Nurse Practitioner (Full-Time / Part-Time)

The Position:
Our client is seeking a Family Nurse Practitioner.
- $140000k W2 (1099 option available)

Requirements:
- Masters degree in Nursing and graduate of an accredited Nurse Practitioner program
- Unrestricted New York State Nurse Practitioner license
- Unrestricted DEA license to prescribe all commonly prescribed medications
- Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid
Responsibilities:
- Monday Friday (8am 5pm)
- No Nights
- No Weekends
- Conduct comprehensive health assessments of residents including physical examinations medical histories and medication reconciliation
- Develop and implement individualized care plans in collaboration with the interdisciplinary team considering the unique needs and preferences of each resident
- Manage chronic conditions and acute illnesses providing timely interventions and appropriate referrals as needed
