Registered Nurse- Case Manager (Contract Role)
Location: Santa Fe NM 87505
Duration: 32-week contract
Shift: 08:00 AM - 04:30 PM
Guaranteed Hours: 40 per week
Weekly Pay Rate: $1920 - $2120; (Negotiable based on experience)
Job Overview:
We are seeking a highly skilled and dedicated RN - Case Manager for a 32-week contract position. The successful candidate will be responsible for coordinating care for patients ensuring they receive comprehensive cost-effective and high-quality services. This is an exciting opportunity to contribute to patient care management while working with a collaborative healthcare team.
Key Responsibilities:
- Conduct comprehensive assessments of patients medical psychosocial and environmental needs.
- Develop individualized care plans and ensure appropriate interventions to address patients needs.
- Coordinate with physicians specialists and other healthcare providers to implement patient care plans.
- Monitor patient progress evaluate effectiveness of interventions and make necessary adjustments to care plans.
- Provide education and resources to patients and families about treatment options discharge planning and community resources.
- Ensure compliance with healthcare regulations policies and standards of care.
- Maintain accurate and up-to-date documentation of patient care plans interventions and outcomes.
- Must have experience in discharge planning to ensure smooth transitions from care settings to home or other care environments.
- Support the interdisciplinary team by communicating key information about patients needs and progress.
- Ensure timely and effective communication with patients families and providers regarding care plans.
Qualifications:
- License: Registered Nurse (RN) with a valid nursing license in the state of New Mexico.
- Experience:
- At least 3 years of clinical nursing experience.
- Minimum 1 year of experience in case management.
- Must have experience in discharge planning.
- Certifications (Required):
- BLS (Basic Life Support)
- Advanced Cardiovascular Life Support (ACLS)
