Orientation $287.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
On Call (Daily) $5700 - Per 24 hours to include 8 hours gratis
24 Hour Holiday Call Rate - $8550 - Per 24 hours to include 8 hours gratis
Call Back $575 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back $862.50 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Average Callback Hours: Weekday: 9.94 hours Weekend: 11.03 hours

- 7 shifts total per month needed from all locums.

- Reason for Coverage: Last minute cancellation.
- Dealbreakers (Do Not Present): Lack of availability for requested dates.
EMR Required Training:
- Credentialing Preapp and full app must be completed and submitted to move provider into applicant in training to generate AD account and begin EMR account set up. Once AD account creation begins it can take up to 5 days to create account.
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Virtual training Only able to attend if Modules are completed before training. Provider will be required to complete Modules EUPA test and PPL at least one week prior to their start date.
- First Day Details - Site requires completion of EMR Modules and training before first day details can be released.
- All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.

Travel Preferences:- Preferred Lodging: Candidates must follow CSH Travel Policy.

- Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:- Timeframe: 60 days standard

- Requirements: Providers must have active CO license and self inquiry NPDB report to release preapplication. If accepted with IMLC provider must apply for CO license at time of acceptance. DEA is required to stay in the state of CO for the full year duration of the assignment. If provider needs a new CO license they must have a DEA to transfer to be considered.
- Requirements: Preapp and full app must be submitted within 72 hours of receipt of initial app to allow enough time for AD account creation for EMR requirements. Once both are submitted provider is moved to applicant in training which initiates AD Account creation for provider EMR account and can take up to 5 days before release of EMR training requirements.

Requirement description :
- Board Certified Neurological Surgery - Required OR Truly BE (0-5 years after residency completion)
- Active CO License - Required or IMLC (LOQ at time of submission)
- Active DEA - Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
- Candidate must be available for the stint detailed in the job description to be considered for ongoing dates - Required
