RN or LPN Home Care

Job Description:
The RN/LPN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching/visit duties to ensure the highest quality of care.

Duties and Responsibilities:
- Performs client visits as needed with authorization by the Division of Senior and Disability Services as well as reassessment visits for In-Home Services and Advanced Personal Care (APC) clients.
- Weekly Medication set up
- Monthly Diabetic Nail care
- General Health Evaluation
- Skin Assessments
- Nursing Assessments
Qualifications Knowledge Skills and Abilities:
- Graduate of approved School of Professional Nursing.
- Current state Registered Nurseor Licensed Practical Nurse license.
- Minimum one (1) year of experience in professional nursing within the last three (3) a minimum of six (6) months of experience with Medicaid In-Home Services nursing.
- Geriatric nursing experience helpful.
