Job Summary

Navitas Healthcare LLC is seeking Registered Nurse (RN) Adult Carefor an exciting job in New York NY.

Responsibilities

Deliver skilled nursing care to adult patients in home and community care settings.
Conduct comprehensive assessments create individualized care plans and monitor patient progress.
Collaborate with physicians interdisciplinary teams patients and families.
Administer medications treatments and prescribed interventions.
Educate patients and families on disease management medications and overall health maintenance.
Maintain accurate and timely documentation in compliance with regulatory and organizational standards.
Work every other weekend as part of the schedule.

Required Skills & Experience

Minimum 2 years of acute care experience (Med-Surg Maternal & Child Care Critical Care or Pediatrics).
Current Adult Care home health experience.

Education & Licensure

Education: Associates Degree (ASN ADN) from an accredited nursing program (BSN preferred).
Licensure/Certifications:
- Active New York State RN License.
- Primary Source Verification of License and Registration with NYSED.
- AHA BLS certification (required).
