Job Summary

Description

We are currently seeking a dedicated and experienced Nurse Practitioner (NP) specializing in Family Medicine . As an NP in Family Medicine you will play a crucial role in providing comprehensive healthcare services to patients of all ages within a family practice setting. This position is vital to our organization as it directly impacts the well-being and health outcomes of our patients. You will be involved in a variety of projects aimed at promoting preventive care managing chronic conditions and fostering patient education.

Responsibilities:

Conduct thorough assessments of patients health status and develop appropriate care plans.
Diagnose and treat common acute illnesses and injuries.
Manage chronic health conditions such as diabetes hypertension and asthma.
Order and interpret diagnostic tests as needed.
Prescribe medications and other treatments following evidence-based guidelines.
Provide patient education on disease prevention healthy lifestyle choices and treatment options.
Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
Maintain accurate and up-to-date medical records.

Requirements:

Masters degree in Nursing with a specialization in Family Medicine.
Active Nurse Practitioner license in the state of practice.
Certification as a Family Nurse Practitioner (FNP).
Demonstrated experience working in a family practice or primary care setting.
Strong clinical assessment and diagnostic skills.
Excellent communication and interpersonal abilities.
Ability to work effectively in a fast-paced environment.
Proficiency in electronic health record (EHR) systems.

Opening Highlights:

Schedule: 4x 10-hour shifts per week
No call requirement
Openings available at multiple locations
Fully-established patient panels
30 minutes per new patient 15 minutes per follow-up
Must be board-certified or board-eligible
