Job Summary

Able2 is currently seeking a professional compassionate reliable and eager LPN for one of our locations.

Duties & Responsibilities:

  • Performs treatments and procedures as ordered by the physician with the understanding of its cause and expected effects.
  • Assists observes and provides training to Direct Support Professionals in all aspects of personal care and hygiene as requested by the RN (transfers nail care oral care dressing toileting dining/feeding bathing changing.)
  • Reviews Care Plans written by the RN for unlicensed DSP understandability.
  • Collaborates with the Residential Med Clerk to schedule and maintain an up-to-date medical appointment calendar (i.e. health related appointments lab work and medical testing). Ensures that completed consults and follow up documentation are uploaded in both Therap and the persons computer medial folder. Attends and assists with appointments as directed by the RN.
  • Observes symptoms reactions lab results and health needs of each person. Accurately records and reports information to the RN and Physician as deemed appropriate.
  • Obtains and documents monthly (or more often as directed or needed) vital signs weights etc.
  • Implements appropriate therapy programs for each person as directed by the Care Plan including but not limited to ROM BART Therapeutic Feeding AFO and splint application.
  • Transcribes signs and administers prescribed medications for each person as assigned. Updates the Medication Administration Records as needed; checks medication deliveries for accuracy and count monitors and reports medication errors per policy.
  • Assists the RN in establishing and maintaining Physician Orders DNR orders and medication histories for each person.
  • Communicates with designated RN daily identifying any unusual incidents or health care concerns; directly observed health changes; providing updates on concerns reported by staff; incidents; changes in individuals health baseline; or any other medical/ nursing concern as outlined in the individualized Care Plan.
  • Attends and participates in individual program planning meetings as requested.
  • Adheres to agency standard of ethical behavior as outlined in the Corporate Compliance Program the Code of Business Conduct and associated policies and guidelines and all applicable laws and regulations.
  • Performs such other duties as assigned by the Nurse Manager or Designated RN.

Qualifications:

  • Must be currently registered as a NYS Licensed Practical Nurse.
  • Physical ability to be on feet all day and perform lifting and supporting of individuals as required by responsibilities listed above.
  • Must be able to obtain and maintain required certifications including CPR and SCIP-R as appropriate.
  • Prefer at least one year experience as a LPN.
  • Must possess a valid drivers license and driving record acceptable to agency insurance carrier.
