Position Summary
A managed care organization serving Ohio Medicaid members is seeking a Licensed Practical Nurse (LPN) to join its Care Management team in a fully remote capacity. This position supports the coordination and facilitation of admissions to long-term care and rehabilitation facilities for members requiring skilled or extended care services. The ideal candidate will have a background in long-term care senior living intake or care coordination along with familiarity with Medicaid eligibility coverage and placement processes.
Key Responsibilities
Support admissions and transitions of care for members entering long-term care skilled nursing facilities (SNFs) or rehabilitation settings.
Collect and review clinical documentation assessments and eligibility information for RN or physician review.
Serve as a liaison between the health plan facility admissions teams discharge planners and providers to ensure timely and efficient placements.
Assist with prior authorization requests documentation preparation and Medicaid eligibility verification.
Communicate with members and families to explain services benefits and what to expect during transitions into care.
Maintain accurate and timely documentation in electronic medical records (EMR) and care management platforms.
Escalate complex cases or clinical determinations to the RN or Utilization Management team as required.
Ensure compliance with HIPAA state regulations and internal protocols.
Qualifications
Active unrestricted LPN license in the State of Ohio (required).
At least 23 years of nursing experience in long-term care rehabilitation or admissions/intake coordination.
Familiarity with Ohio Medicaid processes coverage criteria and facility admissions workflows.
Strong communication documentation and organizational skills.
Proficiency with EMR systems and care coordination tools.
Ability to work independently and remotely in a fast-paced environment.
Preferred Experience
Experience in managed care health plan support roles or insurance case management.
Background in discharge planning transitional care or post-acute intake coordination.
Bilingual English/Spanish is a plus but not required.
