Position Details :
Licensed Practical Nurse (LPN) Memory Care Unit
Schedule: Saturday - Sunday Days 8 hour shifts
Supportive environment where residents and staff are valued
Key Responsibilities:
Administer medications and treatments in accordance with care plans
Oversee daily care provided by CMTs and CNAs
Assist with admissions discharges and care transitions
Communicate effectively with residents families and interdisciplinary teams
Monitor and report changes in resident health status
Job Requirements
