Job Summary

An opportunity to join an innovative trauma-informed team of behavioral health professionals dedicated to helping individuals live their best lives. This role involves providing outpatient intensive outpatient and psychiatric day treatment services in a collaborative care environment. Licensure in social work professional counseling or a related field is required.

Key Responsibilities

  • Complete psychosocial assessments develop treatment plans and monitor progress toward treatment goals.

  • Provide crisis intervention and de-escalation as needed.

  • Deliver trauma-informed individual family and group therapy to diverse populations with varying diagnoses.

  • Serve as an educator to patients and families on mental health and related topics.

  • Coordinate discharge planning and aftercare services.

  • Collaborate with the behavioral health team medical staff and community partners to ensure comprehensive care.

  • Apply diagnostic criteria to evaluate and recommend appropriate levels of care.

Qualifications

Education & Training:

  • Masters Degree in Human Services (e.g. Social Work Professional Counseling or related field).

  • Licensed as LGPC or LMSW (required); LCSW-C or LCPC (preferred).

Experience:

  • Minimum of one (1) year experience working with individuals with acute and chronic mental illness in an acute care psychiatric setting (preferred).

Skills & Abilities:

  • Competence in behavioral health clinical practice including assessments therapy services discharge planning and coordination with community providers.

  • Knowledge of community resources Medicare/Medicaid and commercial insurance.

  • Familiarity with state and federal regulations related to clinical practice.

  • Strong critical thinking communication and time management skills.

  • Ability to work effectively in fast-paced and crisis environments.

  • Flexibility to adapt hours to meet patient and partner needs.
