_{Job Summary:}

_{Physical Therapist (PT) to join our rehabilitation this role you will be responsible for screening evaluating re-evaluating and delivering evidence-based therapy services to both inpatients and outpatients referred by a licensed physician. You will play a key role in promoting optimal functional outcomes for patients ranging from pediatric to adult populations.}

_{Key Responsibilities:}

• _{Conduct initial assessments and develop individualized treatment plans based on clinical findings.}

• _{Provide therapeutic interventions and reassess progress throughout the treatment cycle.}

• _{Demonstrate an understanding of human growth and development for effective care across age groups.}

• _{Collaborate closely with the interdisciplinary healthcare team to coordinate comprehensive patient care.}

• _{Communicate effectively with patients families and caregivers to ensure understanding and participation in therapy goals.}

• _{Maintain accurate and timely documentation in accordance with departmental and regulatory standards.}

• _{Participate in quality assessment and improvement activities as needed.}

_{Qualifications:}

_Required:

• _{Bachelor s degree in Physical Therapy from an accredited program}

• _{Current New Hampshire Physical Therapy License}

• _{BLS Certification (American Heart Association)}

_Preferred:

• _{At least 1 year of clinical experience in a rehabilitation setting}

• _{Strong interpersonal and teamwork skills}

• _{Experience working with both inpatient and outpatient populations}
