Job Description
This is a temp to perm position solely. Candidates who are interested in accepting a permanent role with this facility after a 13 week contract are encouraged to apply. **WA LICSW WA LSWAIC OR WA LASW required upon submittal temporary licensure is accepted** Open to true travelers as well as locals! First time travelers are accepted. This is a part time (24 hours/week) position supporting an Everett Adult Day Care Program. As a member of the interdisciplinary team a candidate in this role provides social work services to participants in a manner which is consistent with the mission and core values of the Health System. Spends a majority of time in direct service activities within the Center including one-on-one and group counseling interaction with other team members and ongoing assistance to participants to address social issues. Based on comprehensive assessments provides perspective on the social emotional cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings including the participants home as needed. A candidate in this position will do some though not all of their assessments in the homes of the participants. It is a moderate to light amount of in-field work and its definitely far less than a home health or hospice social worker would do. This could be within the radius of Snohomish County. A candidate will be reimbursed for mileage and hours driving from one work location to another. This will count as paid time worked (ex: MSW drives from PACE center to participants home for assessment then drives back to PACE center). It is expected that all travel will occur within the workday of 8am-5pm which would all be paid time. Mileage will be reimbursed when the candidate is traveling from one work location to another but the candidates commute from their personal home will not be paid. A candidate in this role will have a lot of autonomy in scheduling their home visits so visits will not be needed to be scheduled in connection with their personal commute. Role: Licensed Social Worker Start Date: ASAP Shift: 3 x 8 Days Years of Experience REQ: 2 years must be with geriatric population First-Timers Accepted: Yes Weekend REQ: No Is on-call REQ No Certs REQ: BLS MSW Then either - WA LICSW WA LSWAIC OR WA LASW. Only one of these social work designations is required. Temporary licensure of any of these types as designated by the state of WA is OK.
