Job Summary
Physical Therapist LTC/SNF Travel Position
Location: Lewistown MO
Job Type: Travel Contract
Duration: 13 weeks
Shift: 3x12s Days
Pay: $2225/week
Job Description
We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Lewistown MO on a travel contract. The Physical Therapist will evaluate residents develop tailored treatment plans and deliver therapy services to enhance mobility restore function and promote independence. The ideal candidate is patient-centered adaptable and experienced in geriatric rehabilitation.
Key Responsibilities
- Conduct thorough evaluations to assess residents functional abilities and therapy needs.
- Develop and implement personalized treatment plans in coordination with physicians and care teams.
- Provide therapeutic interventions including strengthening gait training balance activities and mobility support.
- Educate residents families and caregivers on therapy objectives safety practices and use of assistive equipment.
- Document evaluations treatments and progress notes accurately and promptly.
- Collaborate with nursing and therapy staff to ensure coordinated patient-centered care.
- Support independence safety and improved quality of life through evidence-based therapy practices.
Qualifications
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Missouri (or compact license).
- Minimum of 1 year of recent PT experience preferably in skilled nursing or long-term care.
- Current certification in Basic Life Support.
- COVID-19 vaccine and recent tuberculosis test.
- Strong clinical judgment communication and organizational skills.
- Compassionate approach with experience supporting geriatric populations.
