Job Description
We are seeking a skilled Physical Therapist to join a skilled nursing and long-term care facility in Oklahoma City OK on a travel contract. The Physical Therapist will evaluate residents develop care plans and provide therapy to improve strength mobility balance and overall quality of life. The ideal candidate is compassionate adaptable and experienced in geriatric rehabilitation.
Key Responsibilities
- Perform initial evaluations to determine residents mobility strength and therapy needs.
- Develop and implement personalized treatment plans in collaboration with physicians and interdisciplinary staff.
- Deliver therapeutic interventions including gait training balance exercises strengthening and functional mobility activities.
- Educate residents families and caregivers on therapy goals safe mobility practices and use of assistive devices.
- Accurately document therapy sessions progress notes and evaluations in compliance with facility and regulatory standards.
- Collaborate with occupational therapy speech therapy and nursing teams to ensure comprehensive patient-centered care.
- Promote independence and improved quality of life through evidence-based physical therapy practices.
Qualifications
- Doctorate or Masters degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Oklahoma (or compact license).
- Minimum of 1 year of recent PT experience preferably in LTC/SNF settings.
- Current certification in Basic Life Support (BLS).
- COVID-19 vaccine and recent tuberculosis test.
- Strong communication assessment and documentation skills.
- Compassionate patient-focused approach with geriatric rehab experience preferred.
