Physical Therapist Long-Term Care/SNF - Travel Position
Location: Jackson TN
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s Days
Pay: $2165/week
Job Description
We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Jackson TN on a travel contract. The Physical Therapist will evaluate residents create individualized treatment plans and deliver therapy services aimed at improving mobility restoring function and promoting independence. The ideal candidate is patient-centered detail-oriented and experienced in geriatric rehabilitation.
Key Responsibilities
- Conduct comprehensive evaluations to assess residents functional abilities and therapy needs.
- Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary staff.
- Provide therapeutic interventions including strengthening gait training balance activities and mobility support.
- Educate residents families and caregivers on therapy goals safety practices and use of assistive equipment.
- Document evaluations treatment sessions and progress notes accurately and timely.
- Collaborate with nursing and therapy staff to ensure coordinated patient-centered care.
- Promote safety independence and improved quality of life through evidence-based therapy practices.
Qualifications
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Tennessee (or compact license).
- Minimum of 1 year of recent PT experience preferably in skilled nursing or long-term care.
- Current certification in Basic Life Support.
- COVID-19 vaccine and recent tuberculosis test.
- Strong communication assessment and organizational skills.
- Compassionate approach with experience supporting geriatric populations.
