The Older Adult Behavioral Health Specialist (OABHS) is primarily responsible for the providing interagency multi-system cross-sector planning for better coordination between behavioral health and primary care providing complex case consultation for older adults and for workforce development and community health and wellness promotion in the community. Part of the job as an Older Adult Behavioral Health Specialist QMHP is to provide direct services including evaluation and assessment of the older adult population experiencing behavioral health.
The Older Adult Behavioral Health Specialist coordinates integrated behavioral health treatment services that are based on medical necessity and that support and promote behavioral health and recovery. This position will focus on the older adult population and includes intensive coordination discharge planning and care coordination both in in-patient and out-patient settings. The OABHS will assist clients experiencing high needs high utilization and complex behavioral health care coordination needs in navigating appropriate community services. The OABHS will focus on engaging the client in the treatment process and assisting in accessing appropriate services within the community to meet their treatment goals in order to avoid institutionalization.
This position will work closely with clients providers stakeholders utilization management and Peer Support to develop and promote community health and wellness. The OABHS will be a direct liaison between the client and providers and State stakeholders to assist with education in community-based health and wellness and education of clinical best practices for this population.
The OABHS will promote partnerships and linkages within the community promote the value of collaboration for the wellness of the client and build shared common cultures and values.
The OABHS will adhere to the OARs; agency standards and practices; and follow their board in regard to ethical and standards of practice.
