  • Dunedin Hospital Permanent 0.6 FTE 24hours per week
  • Salary Band $75773 - $106739 p.a prorata KiwiSaver

About the Role

As a Registered Nurse within the Home Team your role is to support discharge from hospital prevent unnecessary hospital admission and provide a rapid response service intercepting patients from st Johns and GP practises. Through rapid nursing assessment with support from the interprofessional team bringing in allied health. This service works in partnership with patients whanau and key stakeholders to provide comprehensive ingrateed services that are responsive to the patient and service needs.

The focus of the service:

  • Hospital prevention and supported discharge
  • Working across a range of settings including the community Emergency Department Internal Medicine Assessment Unit and the in-patient wards.
  • Deliver one car up the driveway one assessment and one plan of care.
  • Support the early discharge of appropriate patients from Dunedin Public Hospital.
  • Monitor patient recovery and ensure everyone receives appropriate rehabilitation and/or packages of care
  • Maximise patient functional ability and maintain their independence in their own home.
  • MondaySunday with staggered start and finish times between the hours of 8 am- 630pm

About you

Within this position you will require:

  • Advanced comprehensive clinical assessment
  • Recent experience of working with the older adults in acute rehabilitation or community settings
  • NZ Registered Nurse with a current APC
  • A full clean NZ driver licence.
  • It is also an expectation that you will have / be working towards a level 3 or 4 portfolio.
