This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. A minimum of two years of experience is required.

Location: Santa Monica California United States.
Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and support staff.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Hospice care (if applicable based on assignment).
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and support staff.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Hospice care (if applicable based on assignment).

- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of hospital home health or hospice care experience.
- Drivers license car insurance and personal automobile for field work (if applicable).
- Medication administration skills.
- Treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.

Preferred Skills & Certifications:
- Experience in multiple nursing specialties (Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
Special Considerations:
- The role may involve working in a fast-paced environment and requires strong collaboration skills. Specific unit assignment (e.g. Medical/Surgical ICU etc.) will be determined.
Scheduling:
- Shift times are variable starting at 7:00 AM or 3:00 PM.
- Weekend work is required every other weekend.
- Weekend work is required every other weekend.
