About the role

We have an opportunity for an experienced Case Worker to work alongside people with disability and mental health needs receiving supports in the Kimberley WA. This role works across both disability Individualised Living Options (ILO) and mental health community engagement supports. The Case Worker will work directly with individuals and their supporters to ensure they receive high quality and safe support that is focused on achieving positive life outcomes. The position will provide supervision to Carers who are supporting people accessing ILO supports and provide direct support to people accessing mental health supports.

This is a permanent position based in Broome. Flexible working hours and arrangements available. A laptop iPhone and Tool of Trade vehicle will be included.

Key Responsibilities

  • Exploration and design of Individualised Living Options
  • Work in partnership with participants families Host Carers and other stakeholders
  • Coordinate supports to meet individualised needs
  • Monitor and review quality of care and deliverables in line with individuals support plans and expectations
  • Build and strengthen Host Carer skills in line with Disability Standards and NDIS Quality and Safeguarding Commission requirements
  • Provide direct support services to people accessing community engagement supports through WA Mental Health Services
  • Maintain an ongoing commitment to improving practice skills and experience by actively participating in any personal and professional development available
  • Complete service outcome reporting and assist the people we support in requesting appropriate individualised support funding at NDIS reviews
  • Some travel is required throughout the Kimberley (typically quarterly)
  • Must participate in on-call roster as required

Skills & Experience

  • Qualification in disability services mental health allied health human services or similar is preferred but not required
  • Demonstrated experience in support planning with people who have a disability or mental health needs
  • Demonstrated experience in working autonomously in implementing support plans for multiple clients simultaneously
  • Understanding of current NDIS frameworks
  • Current unrestricted drivers licence

Successful candidates will be required to clear probity checks including National Criminal History Record Check NDIS Workers Screening Check and Working with Children Check.
