Are you a passionate and dedicated Registered Nurse looking to make a difference in patients lives Jefferson Healthcare is seeking a full-time RN Coordinator to enhance our Home Health & Hospice team. Our program is fully accredited and committed to providing compassionate care that allows our patients to remain comfortably at home and avoid unnecessary hospitalizations. Working in Home Health offers a unique environment where nurses can showcase their skills and truly impact patient well-being.
Key Responsibilities:
- Serve as a vital resource for patients clinical staff non-clinical staff and providers ensuring seamless wayfinding within our services.
- Conduct initial screenings triage assessments and eligibility verifications for potential patients seeking home health hospice and palliative care services.
- Collaborate closely with the Clinical Operations Manager and Business Operations Manager to ensure high-quality patient care aligns with management directives and quality initiatives.
- Build and maintain strong relationships with HHHPC JH staff and referral sources to achieve department and programmatic goals.

What youll need:
- Current Washington state RN license in good standing
- Current CPR/BLS (Healthcare Provider) required
- Bachelor of Science in Nursing preferred
- Home health experience preferred
- Epic experience preferred
Schedule: 1.0 FTE 40 hours/week Day shift
