Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care through assessment diagnosis and treatment of various physical conditions. This role involves working with patients of all ages including pediatrics and geriatrics in both inpatient and outpatient settings. The Physical Therapist will develop individualized care plans implement therapeutic interventions and collaborate with other healthcare professionals to ensure optimal patient outcomes.

Responsibilities

  • Conduct comprehensive evaluations of patients to determine their physical therapy needs.
  • Develop and implement effective treatment plans tailored to each patients condition and goals.
  • Utilize various therapeutic techniques including trigger point therapy physiotherapy and orthopedic rehabilitation.
  • Provide education to patients and their families regarding treatment plans exercises and home care strategies.
  • Document patient progress using EMR systems and maintain accurate medical records.
  • Collaborate with interdisciplinary teams to coordinate discharge planning and ensure continuity of care.
  • Stay updated on the latest advancements in physical therapy practices medical terminology anatomy knowledge and physiology knowledge.
  • Participate in community outreach programs to promote health awareness and preventive care.

Experience

  • Proven experience in physical therapy practice within acute care settings or outpatient clinics is preferred.
  • Familiarity with home health care services and community-based rehabilitation is a plus.
  • Experience working with diverse populations including pediatric and geriatric patients.
  • Strong understanding of anatomy knowledge physiology knowledge medical terminology discharge planning and care plans.
  • Previous hospital experience is advantageous for understanding the dynamics of inpatient care. Knowledge of Home Care Home Base or similar system for documentation and patient management is essential.
