Looking for purpose-driven work with a healthy work-life balance As a Health Care Coordinator - RN youll enjoy the autonomy to shape care the opportunity to make a meaningful impact in the lives of adults with developmental disabilities and the benefit of a regular schedulewithout the long hospital shifts.
Qualifications:
- Must be at least 21 years old.
- Current RN license in good standing issued by the State of New Jersey.
- Minimum 1 year of RN experience.
- Intermediate to advance proficiency in Microsoft Office.
- Demonstrated ability to enter notes for services and billing in an electronic health records system
- Case Management experience
- Experience working with individuals with developmental disabilities and/or mental health population preferred.
- Supervisory experience of CNAs and LPNs preferred.
- Valid drivers license with a good driving record.
- Must own transportation to and from work and between work sites
- Must pass mandatory Division of Developmental Disabilities (DDD) trainings upon hire.
- Must maintain current CPR and First Aid certifications.
Requirements
Key Responsibilities:
- Monitor and advocate for the health and medical care of individuals served.
- Review update and maintain medical documentation and databases (including MyEvolv system).
- Conduct monthly nursing audits and report findings.
- Educate individuals served families and staff on health-related topics and policies.
- Attend and participate in residential staff meetings and administrative meetings.
- Visit assigned residential homes at least three times per week.
- Accompany individuals to complex or serious medical appointments as needed.
- Collaborate with staff to implement Division of Developmental Disabilities (DDD) medical policies and agency protocols.
- Assist individuals in navigating managed care systems and maximizing health benefits.
- Ensure compliance with health and safety standards including PPE requirements.
- Participate in agency trainings meetings and quality assurance activities.
Performance Expectations:
- Dependable attendance and completion of work assignments.
- Clear and effective communication with individuals served staff and external parties.
- Ability to maintain confidentiality of sensitive information.
- Build and maintain positive working relationships across all levels.
Travel Requirement:
- Approximately 75% local travel between assigned homes and facilities.
Work Environment and Physical Demands:
- Exposure to medical hazards including bloodborne pathogens (training provided).
- Must be ambulatory and able to lift up to 50 pounds.
- Able to wear PPE (such as masks) per CDC and OSHA guidelines when required.
- Strong verbal written and computer skills necessary.
Pay Range:
$70000 - $85000
