POSITION SUMMARY

The Speech-Language Pathologist (S-LP) will work as a member of the School-Based Rehabilitation Services (SBRS) team providing speech language therapy services to CHEO patients aged 4 - 21 years of age in a variety of settings including homes schools and other areas as needed. The S-LP will conduct clinical assessments and develop treatment plans and goals in consultation with the patient family and other interdisciplinary team members in accordance with the standards of the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO). The Speech-Language Pathologist provides recommendations for follow-up and management of the patients condition.

This position will provide a range of client-focused interventions for children and their families including but not limited to education assessment individual and group treatment in alignment with the clinical pathways current in place for speech therapy. This role may also support the creation of additional therapeutic pathways. The School-Based Rehabilitation Service supports speech needs including fluency articulation motor speech phonology voice and resonance.

These services may be provided virtually or at one of CHEOs locations across Eastern Ontario or in schools across the Champlain region. An appropriate work from home environment ensuring the safety and privacy of health records and information is required as is a stable internet connection capable of supporting smooth interruption free clinical appointments with families and patients. Due to the hybrid nature travel will be required to various CHEO sites community locations and/or schools. Working hours could vary from 8:00am to 4:00pm or 10:00am to 6:00pm.

LOCATION (S): Travel throughout all of Ottawa may be required. Including the ability to travel to provide service to patients throughout Ottawa Ottawa West (Kanata) Ottawa East (Orleans/Cumberland) Nepean Barrhaven and all points in between.

MAIN RESPONSIBILITIES

  • Completes assessments of patients in a variety of treatment settings including private homes and schools and performs an analysis based on objective subjective and functional findings.
  • Establishes realistic measurable and time limited treatment goals and develops treatment plan.
  • Ensures that speech-language therapy treatments are initiated with the informed consent of the patient or substitute decision maker in accordance with the Health Care Consent Act.
  • Provides direct treatment and/or support and teaching to informal caregivers school personnel and other health care professionals involved.
  • Participates in case conferences when required and consults with other members of the health care team in fostering a holistic approach to care.
  • Completes reassessments to review patients progress and treatment goals and reports to Care Coordinator and other members of the health care team in order to ensure continuity of care and determine if other resources and referrals are required.
  • Plans in collaboration with the patient family and Care Coordinator for the termination of service when patients goals are achieved and/or when patient no longer requires the service.
  • Completes documentation and maintains appropriate patient records in accordance with the standards of College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) and CHEO policies and procedures.
  • Responsible for teaching patient family and other team members the treatment program and management.
  • Acts as a resource for CHEO employees service providers school personnel and community by providing educational programs as requested.
  • Attends and participates actively in team meetings and general staff meetings.
  • Assumes responsibility for maintaining professional knowledge and updated skills.
  • Prioritizes Speech Language Therapy services in accordance with the CHEO policy.
  • Perform work in accordance with the provisions of the Occupational Health and Safety Act and Regulations and all CHEO corporate/departmental policies and procedures related to Occupational Health and Safety.
  • Perform other related duties as assigned by supervisor.

QUALIFICATIONS

  • Masters Degree in Speech-Language Pathology (Essential)
  • Registration with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) (Essential)
  • Minimum two (2) years experience as a Speech-Language Pathologist preferably in pediatrics (Essential)
  • Related experience in a community-based setting or other SBRS Program (Preferred)
  • Experience conducting patient assessments virtually (Preferred)
  • Current First Aid/CPR Certification (Preferred)
  • Excellent time management and organizational skills (Essential)
  • Comfort level and skill in preparing and facilitating therapeutic groups (Essential)
  • Highly level of accuracy and attention to detail particularly with medical documentation (Essential)
  • Excellent communication and interpersonal skills especially while communicating with families patients educators and other partners (Essential)
  • Able to work independently and as part of a team. This role is highly independent (Essential)
  • Valid Ontarios drivers license access to a vehicle and the ability to travel from site to site and to multiple community-based visits on a daily basis is required. (Essential)
  • Ability to travel to provide service to clients throughout all of Ottawa Ottawa West (Kanata) Ottawa East (Orleans/Cumberland) (Essential)
  • Ability to attend last-minute meetings in-person and/or virtually (Essential)
  • Flexible hours including evening weekend and holiday work may be required.

RÉSUMÉ DES FONCTIONS

Lorthophoniste travaille en tant que membre de léquipe des services de réadaptation en milieu scolaire (SRMS) offrant des services dorthophonie aux patientes et patients du CHEO âgés de 4 à 21ans dans divers environnements y compris à domicile à lécole et dans dautres régions au besoin. Lorthophoniste effectue des évaluations cliniques et élaborera des plans de traitement et des objectifs en consultation avec la patiente ou le patient la famille et dautres membres de léquipe interdisciplinaire conformément aux normes de lOrdre des audiologistes et des orthophonistes de lOntario (OAOO). Lorthophoniste formule des recommandations pour le suivi et la prise en charge de létat de la patiente ou du patient.

Ce poste vise à offrir une gamme dinterventions axées sur la clientèle pour les enfants et leur famille y compris sans sy limiter léducation lévaluation le traitement individuel et de groupe conformément aux voies daccès clinique actuelles en orthophonie. Ce rôle peut également appuyer la création de nouvelles voies daccès clinique. Les services de réadaptation en milieu scolaire répondent aux besoins en matière délocution y compris la fluidité larticulation la motricité la phonologie la voix et la résonance.

Ces services peuvent être offerts virtuellement ou à lun des emplacements du CHEO dans lest de lOntario ou dans les écoles de la région de Champlain. Un environnement de travail à domicile adéquat garantissant la sécurité et la confidentialité des dossiers de santé ainsi quune connexion Internet stable pour des rencontres cliniques fluides et sans interruption avec les familles et les patientes et patients est essentiel. En raison de la nature hybride du travail des déplacements seront nécessaires vers différents sites du CHEO ainsi que dans les collectivités et les écoles. Les heures de travail peuvent varier de 8h à 16h ou de 10h à 18h.

EMPLACEMENT(S): Le poste exige de pouvoir se déplacer dans lensemble de la région dOttawa. Notamment des services aux patientes et patients devront être offerts dans louest dOttawa (Kanata) dans lest dOttawa (Orléans/Cumberland) à Nepean Barrhaven et dans les secteurs avoisinants.
