Physical Therapist LTC/SNF Travel Position
Location: Lancaster NH
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s
Pay: $2325/week
Job Description
We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Lancaster NH on a travel contract. The Physical Therapist will evaluate residents create personalized treatment plans and provide therapy services aimed at restoring mobility enhancing strength and promoting independence. The ideal candidate is patient-centered adaptable and experienced in geriatric rehabilitation.
Key Responsibilities
- Conduct evaluations to assess residents functional abilities mobility and therapy needs.
- Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary staff.
- Provide therapeutic interventions including gait training balance activities strengthening and mobility support.
- Educate residents families and caregivers on therapy goals safety strategies and the use of assistive devices.
- Document evaluations treatment progress and outcomes accurately in compliance with facility and regulatory standards.
- Collaborate with nursing and therapy staff to deliver coordinated patient-centered care.
- Support resident independence safety and quality of life through evidence-based practices.
Qualifications
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in New Hampshire (or compact license).
- Minimum of 1 year of recent PT experience preferably in skilled nursing or long-term care.
- Current certification in Basic Life Support.
- COVID-19 vaccine and recent tuberculosis test.
- Strong assessment communication and organizational skills.
- Compassionate approach with experience supporting geriatric populations.
