POSITION SUMMARY

In keeping with the mission statement of the hospital and the patient service units the Care Facilitator is to provide clinical and practice support to the nursing staff. The Care Facilitator will support Sexual Assault team communication education guidance and will be instrumental to the management of SA patients. The position carries with it a commitment to family centered care evidence based practice and professional growth and research.

JOB DESCRIPTION

  • Provides in-depth direct patient care to children youth and families who are experiencing issues related to child abuse and Sexual Assault
  • Provides just-in-time clinical/practical support to SA nurses
  • Fosters the provision of family-centered care by supporting and assisting nursing staff with patient care as deemed necessary and facilitating the advancement of evidence-based practices
  • Acts as a resource person for the SA team staff supporting and promoting efforts leading to the accomplishment of goals team objectives
  • Facilitates and initiates quality improvement activities and participates in PSU quality management program
  • Sets up and maintains various program utilization statistics
  • Updates and develops new policies procedures and medical directives
  • Develops and implements pediatric based SA training activities internally and externally for referring centers
  • Maintains close working relationship with the Child and Youth Protection Program
  • Coordinates follow-up for all SA patients
  • Represents the region on the Provincial Pediatric Committee of the Sexual Assault and Domestic Violence Network
  • Supports local SADV development of protocols and fosters communication and education with the Police Childrens Aid Family Service Agencies and other community programs for children
  • In collaboration with the Clinical Manager and Operations Director ensures on-call coverage is maintained by the SA RN team

QUALIFICATIONS

  • Vulnerable Sector Check (Essential)
  • BScN (Preferred)
  • Current registration with the College of Nurses of Ontario (Essential)
  • Certification in one clinical pediatric specialty (Essential)
  • Training as a Sexual Assault Nurse Examiner (SANE) (Essential)
  • Minimum five (5) years RN experience in a pediatric clinical environment including a minimum of two years experience with pediatric triage charge nurse training and a minimum of two years as a designated preceptor (Essential)
  • Demonstrated leadership skills and abilities (Essential)
  • Demonstrated ability to assign work to monitor work requirements and to deploy resources appropriately and effectively (Essential)
  • Demonstrated ability to assess critical situations quickly to determine action needed and to make effective decisions (Essential)
  • Demonstrated ability to diffuse situations which may be sensitive and/or highly intensive (Essential)
  • Demonstrated ability in working with families during crisis situations (Essential)
  • Must possess highly effective verbal and written communication skills (Essential)
  • Must provide evidence of ongoing professional development and learning (Essential)
  • Knowledge of adult learning principles and previous teaching experience/ability (Essential)

RSUM DES FONCTIONS

Conformment lnonc de mission de lhpital et des units de services aux patients le facilitateur de soins doit fournir un soutien clinique et pratique au personnel infirmier. La facilitatrice / le facilitateur de soins appuiera la communication lducation et lorientation de lquipe des agressions sexuelles (AS) et jouera un rle dterminant dans la prise en charge des patients victimes dAS. Le poste comporte un engagement envers les soins centrs sur la famille la pratique fonde sur des donnes probantes et la croissance professionnelle et la recherche.

DESCRIPTION DU TRAVAIL

  • des soins approfondis et directs aux patients aux enfants aux jeunes et aux familles qui prouvent des problmes lis la maltraitance et aux agressions sexuelles envers les enfants
  • Fournir un soutien clinique et pratique juste--temps aux infirmires du programme des AS
  • Favoriser la prestation de soins axs sur la famille en soutenant et en aidant le personnel infirmier prodiguer des soins aux patients au besoin et en facilitant lavancement de pratiques fondes sur des donnes probantes
  • Agir en tant que personne-ressource pour le personnel de lquipe des AS soutenant et promouvant les efforts menant latteinte des objectifs de lquipe
  • Faciliter et amorcer des activits damlioration de la qualit et participe au programme de gestion de la qualit de lUSP
  • Configurer et tenir jour diverses statistiques dutilisation des programmes
  • Mettre jour et laborer de nouvelles politiques procdures et directives mdicales
  • Dvelopper et mettre en uvre des activits de formation du programme des AS pdiatriques linterne et lexterne pour les centres qui prsentent des demandes de services
  • Entretenir des relations de travail troites avec le Programme de protection des enfants et des adolescents (PEA)
  • Coordonner le suivi pour tous les patients victimes dAS
  • Reprsenter la rgion au sein du Comit pdiatrique provincial du Rseau contre les agressions sexuelles et la violence familiale (ASVF)
  • Soutenir llaboration locale de protocoles dASVF et favorise la communication et lducation avec la police laide lenfance les organismes de services la famille et dautres programmes communautaires pour les enfants
  • En collaboration avec le gestionnaire clinique et le directeur des oprations sassurer que la couverture sur appel est maintenue par lquipe dinfirmires et dinfirmiers autoriss de lquipe des AS
