Position Summary:
- Effectively utilizes hospital resources in the development of care discharge planning and Utilization Review.
- Utilization Management performs prospective admissions observation and concurrent review according to standard policies and procedure.
- Utilization Management reviews and analyze processes designed to ensure appropriate utilization of medication blood products operative and invasive procedures and use of resource intensive diagnostic testing and treatment.
- Discharge Planning assists with discharge planning and processes. Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of the discharge plan.
- Discharge Planning assists with LTC SNF Rehabilitation Behavioral/Mental Health Home Care DME transportation and other placement planning needs.
- Discharge Planning assists with interactions with CPS APS Adoptions and end of life planning and completion of Advance Directives.
- Discharge Planning is a liaison with third-party payers and government programs to access special services for patients.
Minimum Qualifications:
- ADN or BSN required.
- Current licensure applicable to professional affiliation required.
- Certification by the National Association of Healthcare Professionals or InterQual is desirable.
- Current New Mexico licensure as a Registered Nurse.
- Certification as CCM or ACM desired.
