ORGANIZATION OVERVIEW:
Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 1124. We do this work through a comprehensive approach that includes medical mental health housing education and employment youth justice and community programs. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972 Face to Face serves more than 5000 youth annually.
JOB SUMMARY:
The primary role of the Clinic Case Manager is to provide case management care coordination and health insurance enrollment to youth who have medical prenatal and mental health visits at our clinic on the Eastside of Saint Paul. The Clinic Case Manager is responsible for ensuring youth have access and coordination of services to address their needs and improve their overall well-being. This includes assessing needs developing individualized plans and connecting to internal and external support opportunities and resources. The position also assists with tasks outside the direct responsibilities of the role when needed to ensure clinic operations. This is a full-time position at 37.5 hours a week.

PAY & BENEFITS:
The pay rate for this position is between $26.25 and $27.50 based on qualifications.
