Position Summary
The PSH Team Leader will directly supervise Housing Specialist positions a Rental Assistant Manager and a Case Manager. The Team Leader will provide program oversight for the Permanent Supportive Housing Program. The PSH program provides housing support for individuals with serious mental illness who are homeless (as defined by the Department) individuals exiting a state psychiatric facility individuals with serious mental illness released from jail/prison and individuals who may be able to increase their independents by moving from an assisted living facility to permanent supportive housing.
Required Education and Experience
Education Requirements
- Bachelors degree in Human Services (Psychology Social work Counseling or related field)
Types of Experience
- Minimum of two years of related clinical experience and/or the equivalent combination of education and experience that provides the required knowledge skills and abilities. One-year supervisory experience preferred
Related Knowledge Skills and Abilities
- Making sure residences and leases are consistent with HUD requirements.
- Administration of rental assistance along with other financial assistance for setting up a house.
- Linking clients to benefits and resources in the community to maintain housing and placement in the community.
- Maintaining and reporting outcomes to DBHDS
- Processing applications and confirming that the client meets the requirements to be applicable into the program.
- Creating housing support plans
- Knowledge of the principles procedures techniques trends and literature related to mental health developmental disabilities and substance abuse.
- Knowledge of treatment modalities and intervention techniques such as behavior management independent living skills training supportive counseling family education crisis intervention discharge planning and service coordination.
- Knowledge of psychiatric/medical medications/treatments.
- Effectively manage supervisees performance communicate expectations and objectively rate performance of assigned supervisees.
- Knowledge and ability to develop reports create presentations and communicate findings to both internal and external audiences.
- Communicate effectively in order to facilitate treatment and relay information to others as part of an interdisciplinary team.
- Strong organizational skills attention to detail and ability to meet deadlines
- consistently.
- Ability to work independently prioritize multiple tasks and be flexible in job duties & schedule
- Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
- Ability to drive and possess a valid drivers license. May be called upon to travel to any Horizon worksite; transport clients provide services to client @other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites.
- Ability to promote positive collaborative and professional working relations among co-workers and representatives in the community.
- Ability to work in and contribute to a collaborative team environment.
- Ability to respond effectively to changes in the work environment in a positive and professional manner.
- Ability to identify and measure outcomes.
- Willing and available to work modified schedules (weekends holiday etc.) as required.
