Contract Duration: 4 8 weeks (with potential for extension)
Schedule: 5 days/week with possible weekend rotation

Position Overview:

We are seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team in Gloucester this role you will provide high-quality care to residents in a long-term care (LTC) / skilled nursing facility (SNF) environment. Key Responsibilities:

• Deliver direct patient care aligned with individualized care plans
• Administer medications and carry out physician-ordered treatments
• Monitor and document changes in patient conditions
• Support patient admissions transfers and discharges
• Accurately document care in the electronic medical record (EMR) system
• Collaborate with the nursing and interdisciplinary team to meet resident care goals
• Maintain a clean safe and supportive environment for residents

Requirements:

• Active LPN license in the state of Massachusetts
• CPR/BLS Certification (current and valid)
• Minimum 1 year of recent experience in a LTC/SNF setting
• Reliable and flexible to work assigned shifts including weekends
• Strong clinical and communication skills
• Detail-oriented with a commitment to quality care
