PATH is seeking candidates passionate about helping others make a positive change in their lives to join our ICMS Special Programs Team as the LCSW Clinician working out of our main office on Madison Avenue in Los Angeles.

ABOUT PATH
Since its foundation in 1984 PATH (People Assisting the Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.

ABOUT OUR SUPPORTIVE HOUSING SERVICES
Our Supportive Housing Services (SHS) programs provide Intensive Case Management Services (ICMS) in a Permanent Supportive Housing (PSH) setting to individuals who have experienced homelessness and have a history of chronic health mental health and/or co-occurring conditions. Services are centralized and provided to clients living on-site to ensure ease of access to the resources necessary for long-term housing stabilization improving their quality of life and community integration.

ABOUT THE LICENSED CLINICIAN
As part of the ICMS Team the Licensed Clinician provides direct support to the Program Manager as well to their case management colleagues. They will work to provide case management oversight to other case managers who provide supportive services linkage and stabilization services to participants. The participants may be exiting from hospitals emergency rooms and other health care settings.

Position Responsibilities include:

• Provide clinical support training and supervision to program staff including clinical supervision for Board of Behavioral Services registered ASW AMFT and APCC providers accruing hours toward licensure. This position is expected to be available for clinical consultation and regular supervision. Be knowledgeable and have experience providing evidenced based practices effective when serving a vulnerable homeless population including but not limited to Motivational Interviewing Critical Time Interventions Harm Reduction Housing First and Crisis and Suicide Assessment.
• Process and accept referrals from funders.
• Meet with assigned veterans a minimum of 1x/week to support with progression towards goals identified by a guest in their ISP.
• Facilitate psychosocial rehabilitation groups for veterans as required.
• Conduct intake and individualized needs assessment for all veterans and assist the team by collaborating with veterans to develop Individualized Service Plans (ISP) that address barriers to obtaining services housing and appropriate health care needs.
• Conduct a full VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) at admission for all new veterans to aid in the Individual Service Plan (ISP) development.
• Conduct and complete all new veterans intakes and enter information into HMIS in a timely fashion.

PREFERRED QUALIFICATIONS

• Experience working with active service members or veteran populations.

MINIMUM QUALIFICATIONS

• Current and active licensure with the California Board of Behavioral Sciences (e.g. LCSW LMFT LPCC).
• One (1) year of experience as a Clinical Supervisor in a program(s) that serves populations experiencing homelessness and/or populations suffering from comorbidity.

MANDATORY REQUIREMENTS
Regardless of qualifications candidates must:

• Have employment eligibility verification
• Have or be able and willing to obtain CPR/First Aid training
• Successfully complete the following as a condition of hire:
  • Tuberculosis Test
  • Background Screening
  • Drug Test
