Location: Laconia New Hampshire
Position Type: Part-Time or Full-Time
***Sign on bonus$5000 ($2500 paid at 90 days and $2500 paid at 6 months)

In this role youll deliver personalized therapy that supports patients in performing daily activities safely and confidently. If youre passionate about functional outcomes and patient-centered care this is an opportunity to do your best work.
What Youll Do
- Provide in-home occupational therapy services to patients and when applicable in assisted living settings
- Evaluate patients physical cognitive and environmental needs
- Develop and implement individualized care plans in collaboration with the healthcare team
- Help patients improve daily living skills mobility and overall independence
- Recommend adaptive equipment and home modifications to enhance safety
- Educate patients and caregivers on treatment plans exercises and long-term wellness strategies
- Document patient progress and adjust care plans as needed
What Were Looking For
- Active Occupational Therapist license in the State of New Hampshire
- Graduate of an accredited Occupational Therapy program
- Minimum of 2 years of clinical experience (home health preferred)
- Current CPR certification
- Strong clinical judgment communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
