Our role is also supporting people to self-manage long-term health conditions such as diabetes and mental health challenges with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people including older people and people with disability to stay living at home longer and to live well in their community.

Our broad range of primary health services care management and in home and community support services can be tailored to an individuals needs and with major hubs in Box Hill and Eltham these services can be delivered locally in the Eastern and North Eastern suburbs of Melbourne.

Description

The opportunity:
We are looking for a Speech Pathologist to join our supportive and professional multidisciplinary teams in a flexible part time capacity.

Responsibilities include but are not limited to;

  • Provision of high-quality Speech Pathology services to all healthAbility clients from various funding sources.
  • Understand the business rules for HACC CHSP CH ICD MBS HCP and NDIS and achieve targets in all areas. This may include new future funding areas to the organisation.
  • Provide relevant assessment and progress reports to the relevant funding partners in a timely manner.
  • In collaboration with the internal In-Home Support TeamSupport Coordination and Home Care Packages Team identify organise plan implement and evaluate evidence-based services.
  • Work closely with Support Coordinators LAC/NDIAPlanners or Care Managers etc to identify and appropriately manage participant needs.
  • Actively involve clients and/or carers in setting goals and making decisions about their care.
  • Participation in continuous improvement and planning processes.
  • Ensure all clinical service delivery records and funding applications meet all requirements for funding bodies.
  • Contribute to the development and innovation ofintegrated multidisciplinary service delivery models.
  • Participate in clinical supervision and ongoing professional development.


Skills And Experiences

Our ideal candidate:

  • At least 1 year of clinical experience in relevant areas
  • Demonstrated ability to provide customers with an exceptional service experience
  • Ability to work with a high level of independence and autonomy and accountability and able to escalate to senior staff for support when required
  • Ability to develop maintain effective client centredtherapeutic relationships with clients families and carers
  • Demonstrated experience in providing services in both individual and group-based settings
  • Demonstrated ability to work effectively in a team and to foster teamwork
  • High developed interpersonal skills with sound communication and literacy skills
  • Motivated and willing to thrive
  • Demonstrated ability to manage workload and meet specified targets
  • Victorian Drivers License
