DescriptionProvides medication management services in coordination with other health care professionals to deliver safe appropriate and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens medication therapy management medication order verification/entry medication distribution medication therapy monitoring and consultation documentation of medication therapy plans and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally pharmacists are active members in quality improvement research activities and education as described in the job specific competencies.
QualificationsRequired Qualifications
- Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.
Additional Qualifications
- Other advanced degrees (e.g. Masters Ph.D.) and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor preferred.
- Completion of pharmacy residency and specialization (e.g. PGY-2 and respective BPS certification) if applicable preferred.
- Evidence of ongoing scholarly pursuits including research/quality improvement publications or scientific presentations preferred.
- Relevant applicable pharmacy experience working in a progressive and collaborative practice environment preferred.
- Experience and/or interest in teaching and/or precepting preferred.
- Commitment to excellence accuracy attention to detail and teamwork required.
- Ability to communicate effectively work collaboratively with others organizes time well practice with an agile mindset solve problems and work independently with minimal supervision required.
License and Certification
- Current pharmacist license by the Minnesota Board of Pharmacy or eligible to obtain within a time frame compliant with the sites applicable employee licensure policy. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
