POSITION SUMMARY:

The RN Diabetes Care Specialist specializes in the care and education of patients with diabetes. The RN Diabetes Care Specialist has responsibility for providing assessment care planning specialty education and patient evaluation and re-assessment based on the plan of care and for the identified population(s) served. The RN Diabetes Care Specialist provides staff development for case management staff collaborates with case managers care coordinators and physicians. The RN Diabetes Care Specialist participates with coordinating the care and services for assigned patients for an episode of illness or treatment in collaboration with the patient family physician patient care team and payors. The RN Diabetes Care Specialist performs work in multiple acute care (Esplanade Rehabilitation Behavioral Health) settings and utilizes advanced clinical skills to facilitate the provision of care which includes the assessment planning intervention and evaluation which focus on and uses the key principles of the nursing process. The RN Diabetes Care Specialist promotes and evaluates the effective utilization of resources using current clinical knowledge awareness of community services. The RN Diabetes Care Specialist actively participates in a consultative role with the patient care team to achieve optimal clinical and resource addition the RN Diabetes Care Specialist may be assigned to participate on the Diabetes Committee Policy & Procedure development review and work-groups which promote on-going up to date information based on their specialty. The RN Diabetes Care Specialist may be assigned to assist in the outpatient clinic as needed. Advocates for the patient and family throughout the entire episode of care.

EDUCATION / TRAINING / EXPERIENCE:

Minimum:

  • Three years of recent acute care clinical experience OR one year of recent Diabetes Educator or Case Management experience in an acute or post acute setting.

Desired:

  • Bachelors degree in nursing
  • Experience in acute diabetes education and/or clinical case management.
  • Working knowledge of managed care requirements and supply procurement and coverage for diabetes patients.
  • Experience in area of assignment.

LICENSES / CERTIFICATIONS:

Minimum:

  • Current licensure with the California Board of Registered Nursing
  • Certified Diabetes Educator (CDE) or must complete certification process within 6 months.
  • Current CPR recognition

SKILLS / KNOWLEDGE / ABILITIES:

Must be able to demonstrate understanding of diabetes pathophysiology treatment and standards of care (as demonstrated on the diabetes competency module). Must be able to maintain confidentiality of patient information and be a patient advocate. Must have excellent collaboration skills and work cooperatively as a team. Must have initiative to facilitate care and participate with process improvement. Demonstrate decision making skills which produce cooperative effort consideration of both the care team and patient and exhibit conservation of resources and financial stewardship. Assessment planning intervention and evaluation of patient care. Demonstrates current clinical nursing knowledge as evidenced by completion of staff nurse OCAT and passing medication administration examination. Must demonstrate technical and clinical competence in diabetes management. Maintains and regularly updates clinical nursing knowledge by completion of the Skills Fair on an annual basis. Understanding of third party reimbursement methodologies. Critical thinking and analytical skills. Must have excellent communication and interpersonal skills to establish a high degree of rapport and professional interaction. Must be able to maintain strict confidentiality at all times. Ability to build positive working relationships with physicians and external agencies. Ability to communicate with multiple levels of the organization (e.g. managers physicians clinical and support staff). Capable of using (Microsoft Office Suite) Microsoft Word Excel Outlook Midas Chart Max and EPIC computer systems. Organizational and multi-tasking skills are essential. Must be able to fulfill the essential functions of the position.
