Description

We are currently seeking a compassionate and dedicated Nurse Practitioner (NP) specializing in Hospice and Palliative this role you will play a crucial part in providing quality end-of-life care to patients ensuring their comfort and dignity during challenging times. You will have the opportunity to work on a variety of projects focused on improving the quality of life for individuals facing serious illnesses. This position is ideal for individuals passionate about palliative care and making a difference in patients lives.

Responsibilities:

  • Conduct comprehensive assessments of patients physical emotional and spiritual needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Manage symptoms such as pain nausea and shortness of breath through appropriate interventions.
  • Provide education and support to patients and their families on end-of-life care options.
  • Coordinate care with other healthcare providers to ensure holistic patient care.
  • Document patient care activities accurately and timely in electronic health records.
  • Participate in regular team meetings to discuss patient progress and care plans.
  • Adhere to all regulatory standards and guidelines related to hospice and palliative care.

Requirements:

  • Active Nurse Practitioner (NP) license in the state of practice.
  • Certification in Hospice and Palliative Care preferred.
  • Strong communication and interpersonal skills.
  • Ability to provide compassionate and empathetic care to patients and families.
  • Knowledge of pain and symptom management in palliative care.
  • Experience working in end-of-life care or hospice settings is a plus.
  • Proficiency in electronic health record systems.
  • Ability to work collaboratively in a multidisciplinary team environment.

About the Position:

Schedule: Monday-Friday (8a-5p) or 4x 10-hour shifts

No call

Setting: Outpatient

Daily Census: 10-20 patients

Will travel to different homes facilities and clinics

APPs practice independently with some supervision

EMR: AthenaHealth

Board certification required

Preferred Experience: 2 years
