Job Summary

We are searching forcompassionate and energetic individualsto join our team! We are seeking aRN -Patient Navigator.

Responsibilities:
  • Collaborate with providers care managers and interdisciplinary teams to ensure continuity of care across settings
  • Support care transitions (hospital to home rehab to primary care) to reduce readmissions and ER utilization
  • Receives referrals and medical records to coordinate plan of care with medical team (surgery and non-operative)
  • After physician review coordinates plans of care and travel with third party company
  • Close follow up and triage with the patient requiring an on call status
  • Provides patient hand off to hospital or ASC
  • Complies with all our processes and HIPAA regulations.
  • Applies our values in decision-making and interactions with all individuals.
  • Promotes a positive work environment through effective teamwork.
  • Other duties as assigned by changing needs patient flow or physician request.
  • Participates in department or system wide improvement plans
  • Performs high quality work that is neat accurate complete and on time
  • Offers assistance to all patients by promptly responding to requests and needs
  • Asks questions to gain full understanding of assignments
  • Adapts to changing assignments and responsibilities

A Valid NC RN License is required
Hours: Full-time Salaried Mon-Fri
Hours will vary dependent upon patient needs.
This is not a remote position.
