As a Home Health Licensed Practical Nurse heres a big-picture view of what youll do:

• Provide home-based care to patients with illnesses injuries and chronic diseasesoften after hospital dischargeunder the supervision of an RN
• Work with a team of physicians RNs CNAs and aides to deliver quality patient care
• Take patient vitals do assessments document progress and report changes to RN supervisor
• Assist RN in carrying out patients plan of care and teaching patient self-care techniques
• Educate family on the patients plan of care medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

Specifics to the case that we are seeking to staff is a 22 year old male client located in NorthPort. This clienthascerebral palsy and a trach in place. We are seeking to cover the overnights to support the family from 7pm-7am- ranging up to 36 hours per week. The agency will provide on-site training for extended education rated between $30-$32/hr.

A few must-haves for Home Health Licensed Practical Nurses:

• Graduate of an accredited practical nursing program and active LPN license in FL
• Minimum of 1 yearof LPN experience ideally in home healthcare or completion of offices preceptor program
• CPR certification TB test Background check
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing strong communication skills and compassion
