Job Summary
Physical Therapist (PT) LTC/SNF - Travel Position
Location: Stamford CT
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s
Pay: $2095/week
Job Description:
We are seeking a compassionate Physical Therapist (PT) to join a skilled nursing and long-term care facility in Stamford CT on a travel contract. The PT will evaluate plan and deliver therapy services to residents focusing on improving mobility restoring function and enhancing overall quality of life. The ideal candidate is patient-centered detail-oriented and experienced in geriatric rehabilitation.
Key Responsibilities:
- Conduct thorough patient evaluations to assess mobility function and therapy needs.
- Develop and implement individualized treatment plans in collaboration with physicians and care teams.
- Provide therapeutic interventions including strengthening gait training and balance activities.
- Educate residents families and caregivers on therapy programs safe mobility and the use of assistive equipment.
- Document evaluations progress notes and treatment outcomes in compliance with facility and regulatory standards.
- Collaborate with interdisciplinary teams to ensure coordinated care and optimal patient outcomes.
- Promote patient independence safety and quality of life through evidence-based therapy.
Qualifications:
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Connecticut (or compact license).
- Minimum of 1 year of PT experience preferably in long-term care or skilled nursing facilities.
- Current Basic Life Support (BLS) certification.
- COVID vaccine and recent TB test.
- Strong communication assessment and organizational skills.
- Compassionate patient-focused approach with experience supporting geriatric populations.
