Job Description
Physical Therapist (PT) LTC/SNF - Travel Position
Location: Danbury CT
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s
Pay: $2095/week
Job Description:
We are seeking a skilled and compassionate Physical Therapist (PT) to join a skilled nursing and long-term care facility in Danbury CT on a travel contract. The PT will assess residents design individualized treatment plans and deliver therapy services that promote mobility independence and improved quality of life. The ideal candidate is patient-focused organized and experienced in geriatric rehabilitation.
Key Responsibilities:
- Conduct detailed evaluations to assess patient mobility function and rehabilitation needs.
- Develop and implement individualized treatment plans in collaboration with physicians and care teams.
- Provide therapeutic interventions including strengthening balance training gait training and functional mobility activities.
- Educate residents families and caregivers on therapy goals safe mobility techniques and use of assistive equipment.
- Document evaluations progress notes and outcomes in accordance with facility and regulatory requirements.
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient-centered care.
- Support residents in achieving greater independence safety and well-being.
Qualifications:
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Connecticut (or compact license).
- Minimum of 1 year of PT experience preferably in skilled nursing or long-term care facilities.
- Current Basic Life Support (BLS) certification.
- COVID vaccine and recent TB test.
- Excellent communication clinical assessment and organizational skills.
- Compassionate approach with experience supporting geriatric populations.
