IMC is currently hiringaRegistered Nurse - Care Manager who under the generalsupervision of the Unit Director and with the authority of the Chief NursingOfficer will apply nursing process (assessment planning intervention andevaluation) in caring for patients. Effectively interact with patientsignificant others and other heath team members. Demonstrates knowledgeand skills necessary to provide care based on physical psycho/socialeducation safety and related criteria for the newborn pediatric adolescentadult and geriatric population.
EDUCATION
- Graduate of state approved School of Nursing.
LICENSURE/CERTIFICATIONS
- Current licenses as a RN in the state of Louisiana.
TRAINING/EXPERIENCE
- Previous experience preferred but not required.
RESPONSIBILITIES
- Be available by telephone during workinghours
- Act as liason between patientsphysicians and referral sources.
- Be knowledgeable of all assignedpatients.
- Conduct on-going clinical reviews toensure appropriate level of care and necessity of continued stay.
- Reports clinical reviews to third partypayer sources as required/requested.
- Post authorizations into computer asneeded.
- Make appropriate referrals as orderedper MD.
- Ensures that the most effective andlevel appropriate discharge plan is in place for all assigned patients to promotesafety in the post-acute setting.
- Actively involved in interdisciplinaryrounding meetings.
- Complies with all complianceexpectations.
- Knowledgeable of all CMS Mandated Forms.
