Job Description

Physical Therapist (PT) Long Term Care/SNF -Travel Position

Location: Twin Falls ID
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s Days
Pay: $2305/week

Job Description:

We are seeking a skilled and compassionate Physical Therapist (PT) to join a skilled nursing and long-term care facility in Twin Falls ID. The PT will be responsible for assessing planning and implementing therapy programs designed to help patients improve mobility strength and functional independence. The ideal candidate is patient-centered detail-oriented and experienced in geriatric care.

Key Responsibilities:

  • Perform comprehensive patient assessments to evaluate physical function and rehabilitation needs.
  • Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary teams.
  • Provide therapeutic interventions including exercises manual therapy gait training and balance activities.
  • Educate patients families and caregivers on therapy plans home exercise programs and safety strategies.
  • Accurately document evaluations treatment sessions and patient progress in compliance with facility and regulatory standards.
  • Support patients in achieving optimal functional outcomes and enhanced quality of life.
  • Maintain a safe treatment environment and adhere to infection control policies.

Qualifications:

  • Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
  • Current and valid Physical Therapist license in Idaho (or compact license).
  • Minimum of 1 year of experience as a Physical Therapist preferably in long-term care or skilled nursing facilities.
  • Current Basic Life Support (BLS) certification.
  • COVID vaccine and recent TB test.
  • Strong clinical assessment communication and organizational skills.
  • Compassionate and patient-focused approach with experience caring for geriatric populations.
