Hospital Pharmacist - Travel Position
Location: Wichita Falls TX
Job Type: Travel Contract
Shift: 5x8s Weekdays with every third weekend required
Pay: $2681/week
Required Certifications & Competencies
-ACLS (Advanced Cardiac Life Support) AHA Instructor Provider or RQI acceptable
-BLS (Basic Life Support) AHA Instructor Provider or RQI acceptable
-PALS (Pediatric Advanced Life Support) AHA Instructor Provider or RQI acceptable
-NRP (Neonatal Resuscitation Procedure) Required as applicable
-CHEMO (Chemotherapy) Required as applicable
-TNCC (Trauma Nursing Core Course) Required as applicable
-EFM (Electronic Fetal Monitoring) Required as applicable
-ENPC Required as applicable
-NIH Stroke Certification Required as applicable
-Medication Test/Pharmacology Competency Exam Must meet or exceed passing score
-Competency Exam Must meet or exceed passing score
-Tech Cert Verification (National) Required within 30 days of start
-Orientation Safety Manual Acknowledgement Required update annually
-State License Verification Required within 30 days of start
-NURSYS Verification (RN LPN/LVN AP) Required within 30 days of start

-I9 or Attestation
-Government Issued Photo ID (Drivers License/State ID)
-E-Verify or Attestation
-Background Check Within 1 year of start renewed annually
-EPLS/GSA/SAM Required within 30 days of start
-HHS/OIG Required within 30 days of start
-2 References within 3 years of start
-Employment Application/Resume
-Skills Checklist Required update annually
Health & Immunization Requirements
-Physical Exam Within 1 year of start must be completed within 72 hours of contract signing
-Tetanus/Diphtheria/Tdap Vaccine within 10 years of start
-Negative TB Test or Chest X-Ray Within 1 year of start; chest X-ray within 5 years if positive PPD
-TB Questionnaire Required for all candidates
-Rubeola (Measles) 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
-Mumps 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
-Rubella 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
-Hepatitis B Positive titer series or declination required
-Varicella 2 vaccines positive titer or history of chickenpox required
-Flu Seasonal Vaccination Required Sept-May; medical/religious exemptions subject to facility review
-COVID-19 Vaccine

-Flu Religious Exemption (if applicable)
-Parking Policy Acknowledgment Must be completed prior to start
-Drug Testing Negative 9 Panel Drug Screen within 1 year of start; plan for completion within 72 hours of contract signing
