Hospital Pharmacist - Travel Position

Location: Wichita Falls TX

Job Type: Travel Contract

Duration: 13 weeks

Shift: 5x8s Weekdays with every third weekend required

Pay: $2681/week

Required Certifications & Competencies

• -ACLS (Advanced Cardiac Life Support) AHA Instructor Provider or RQI acceptable
• -BLS (Basic Life Support) AHA Instructor Provider or RQI acceptable
• -PALS (Pediatric Advanced Life Support) AHA Instructor Provider or RQI acceptable
• -NRP (Neonatal Resuscitation Procedure) Required as applicable
• -CHEMO (Chemotherapy) Required as applicable
• -TNCC (Trauma Nursing Core Course) Required as applicable
• -EFM (Electronic Fetal Monitoring) Required as applicable
• -ENPC Required as applicable
• -NIH Stroke Certification Required as applicable
• -Medication Test/Pharmacology Competency Exam Must meet or exceed passing score
• -Competency Exam Must meet or exceed passing score
• -Tech Cert Verification (National) Required within 30 days of start
• -Orientation Safety Manual Acknowledgement Required update annually
• -State License Verification Required within 30 days of start
• -NURSYS Verification (RN LPN/LVN AP) Required within 30 days of start

• -I9 or Attestation

• -Background Check Within 1 year of start renewed annually
• -EPLS/GSA/SAM Required within 30 days of start
• -HHS/OIG Required within 30 days of start

Health & Immunization Requirements

• -Physical Exam Within 1 year of start must be completed within 72 hours of contract signing
• -Tetanus/Diphtheria/Tdap Vaccine within 10 years of start
• -Negative TB Test or Chest X-Ray Within 1 year of start; chest X-ray within 5 years if positive PPD
• -TB Questionnaire Required for all candidates
• -Rubeola (Measles) 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
• -Mumps 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
• -Rubella 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
• -Hepatitis B Positive titer series or declination required
• -Varicella 2 vaccines positive titer or history of chickenpox required
• -Flu Seasonal Vaccination Required Sept-May; medical/religious exemptions subject to facility review
• -COVID-19 Vaccine
