Screening and Navigation Coordinator

Are you passionate about making a difference in the lives of individuals with intellectual and developmental disabilities Arc of Onondaga is looking for a dedicated and ambitious professional to join our team as a Screening and Navigation Coordinator.
In this newly created full-time role youll work closely across agency departments to support individuals through the screening process help them navigate services and provide ongoing case management as appropriate. As the primary point of contact youll play a critical role in overseeing the roll out of the new screening tool completion of required billing documentation and creation of internal processes. You will be key to ensuring that the health-related social needs of individuals are met and that no one falls through the cracks.

Job Requirements:
Bachelors degree in Human Services (or related field)
Experience working with individuals with developmental disabilities
Valid NYS drivers license with a driving record acceptable to the agencys insurance carrier
